I don't think it was ever "believe all women". The correct phrase was "believe women". "believe women" is a slogan, like you have described, that is meant counter the cultural tendency to discredit women reporting harrassment or rape. "believe all women" is a adaption of that slogan that means believe all women all the time. It was created to form a straw man argument against the #MeToo movement.
